I have tried starting a project from the "Blank Node.js Console Application" and "Blank Node.js Web Application" projects. When I place a breakpoint and then press F5, the application is executed, but no breakpoints are ever hit. Intellisense is working fine. I am using VS Ultimate 2013 Update 4.
Comments: Just want to make sure we're seeing the same issue here. Can you try creating a starter Express project? Do you hit the breakpoint on the index route when you refresh the page? It seems like this issue only occurs for breakpoints in the initialization steps of the app (it sets the breakpoints fine, but just doesn't set them soon enough to be hit)
